Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home and facility-based hospice care. You will assess patients, implement plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive support for patients and families.

You will educate caregivers, document care meticulously, and participate in on-call duties while adhering to regulatory standards and best practices.

Qualifications

• Must hold current nursing license in the state of employment
• CPR certification required
• Minimum 1 year RN experience (hospital, oncology, long-term care, hospice or home health)
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person and telephonic RN assessments
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized care plans with visit frequency
• Coordinate care across interdisciplinary teams
• Educate patients, families, and caregivers about disease, care and medications
• Communicate changes in condition to the care team
• Maintain timely clinical records and ensure infection control and compliance
