Job summary

Fortis Health is seeking a dedicated Hospice Registered Nurse Case Manager to coordinate home-based patient care from admission through discharge. The role requires assessing needs, developing plans, and ensuring continuity of care under physician orders, while upholding regulatory standards.

You will collaborate with physicians and the interdisciplinary team, manage a caseload, and maintain compliant documentation with a focus on patient safety and quality outcomes.

Qualifications

• Active RN license in the state where the position is posted.
• Minimum of one year of nursing experience in hospital, facility, home health, or related setting.
• Home health and hospice experience preferred.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to work independently in patient homes and manage a flexible schedule.

Responsibilities

• Complete admission assessments and ongoing patient evaluations in the home setting.
• Develop and update patient plans of care in collaboration with physicians.
• Collect and document OASIS data accurately and on time.
• Provide skilled nursing care while managing multiple patients as assigned.
• Assess, manage, and educate patients and families on pain, medications, and disease processes.
• Coordinate care with physicians, interdisciplinary team members, and agency staff.
• Participate in case conferences and communicate changes in patient condition as needed.
• Maintain timely, accurate clinical documentation and comply with policies and regulations.
