Job summary

UnitedHealth Group's At Home Hospice is seeking a dedicated Hospice Registered Nurse Case Manager to deliver the Plan of Care for each patient in their home. You will evaluate care, collaborate with professionals, and implement evidence-based strategies to support physical, psychosocial, and spiritual well-being.

As a full-time role with standard hours Mon-Fri 8:00-4:30, there is occasional on-call rotation evenings/weekends and travel within Harris County to provide in-home patient care.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred: 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility.
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG for daily care.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes regarding the patient's condition.
• Function in all home environments and provide development and direction as needed.
