Beacon Hospice, a part of Amedisys Optum and UnitedHealth Group family, is seeking an experienced Hospice Registered Nurse Case Manager in New London county, CT. You will coordinate compassionate hospice care, assess patients, collaborate with the interdisciplinary care team, and develop care plans tailored to each patient’s needs.

The role requires a current RN license in CT, CPR certification, a valid driver's license, reliable transportation, and at least 1 year of nursing experience.

Qualifications

• Current RN license, CPR certification, and valid driver’s license.
• 1+ years of nursing experience required.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies
• Records pain/symptom management changes/outcomes as appropriate
• Teaches patients, families, and caregivers appropriate patient care techniques
• Regularly communicates with clinical manager and care team
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Other duties as assigned
