Job summary

Priority Dispatch Corp. is seeking a skilled RN to administer nursing care in patients' homes and coordinate with the interdisciplinary team. The role requires adapting plans to patient needs, educating families, and ensuring high-quality end-of-life care with proactive communication.

Responsibilities include comprehensive home assessments, plan development, and on-call availability. A valid RN license and driver’s license are required, with hospice experience preferred and English literacy

Qualifications

Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state license as a Registered Nurse (RN) and/or in accordance with Nurse Licensure Compact.
Current driver’s license.
Two years RN experience in a clinical care setting; Hospice experience preferred.
Read, write and comprehend English.

Responsibilities

Initial and ongoing comprehensive assessments of patient needs in the home.
Develop and coordinate the plan of care with the IDG/Team to ensure quality and continuity.
Educate patients and families on disease process, self-care, end-of-life care, nutrition, and dietary needs.
Provide on-call services and effective communication with patients, families, and health care professionals.
