Job summary

Alive Hospice in Nashville, TN seeks a Registered Nurse for the weekend shift to provide after-hours nursing visits and triage support. Must be available Friday and Saturday evenings, with additional weeknight hours possible.

The role ensures timely nursing care, death procedures, and coordination with the Triage Nurse and Primary Nurse to support the patient care plan.

Qualifications

• RN license current; BSN preferred.
• Ability to apply for TN license within 30 days of hire.
• Knowledge of hospice care; willingness to learn.
• Minimum one year nursing practice or equivalent experience.
• Hospice, Oncology, Home Health or long-term care experience preferred.

Responsibilities

• Assumes after-hours triage role as directed by Triage Nurse.
• Performs home visits to provide skilled nursing care per care plan.
• Completes death procedures and visits at time of death.
• Documents accurately and timely; assumes at least 4 shifts per month.
• Communicates patient status and interventions to Triage and Primary Nurses.
• Interacts with physicians as needed.
• Supports care plan, outcome management and variance analysis.
• Attends meetings as requested.
• Other duties may be assigned.

Registered Nurse - Weekend Shift

Location: Nashville, TN and surrounding areas
Status: Full Time
Days: Must be available to work Friday and Saturday, 5 p.m. - 8 a.m. Additional night hours available during the week

Summary

Provides on call nursing visits as directed by the Triage Nurse to ensure appropriate, timely and effective nursing care after hours.

Essential Duties and Responsibilities

• Assumes after hours triage role if indicated by Triage Nurse due to workload/geography.
• Performs home visits as dictated by the Triage Nurse to provide skilled nursing care at patient's residence in support of patient's care plan developed by Primary Nurse and M.D.
• Makes home visits at time of death to support family and complete death procedure per agency protocol.
• Completes accurate and timely documentation as required, assumes a minimum of 4 shifts per month.
• Communicates patient's status and interventions to Triage Nurse and Primary Nurse.
• Interacts with physician as needed.
• Supports care plan, outcome management and assists in variance analysis.
• Attends meetings as requested.
• Other duties may also be assigned.

Requirements

Education and/or Experience Registered Nurse with a current nursing license, BSN preferred. Ability to apply for TN license within 30 days of hire. Knowledge of or willingness to learn hospice care. Minimum one year nursing practice or equivalent experience required. Hospice, Oncology, or Home Health and/or long-term care preferred.

Certificates, Licenses, Registrations Current Tennessee RN License. If required to drive to carry out the duties of this position: current driver's license and automobile insurance as required by Tennessee State Law.
