Job summary

Affinity Health Management seeks a qualified Registered Nurse for hospice care, focusing on home- and community-based services. You will assess patients, develop and adjust care plans, administer medications, and collaborate with families and the interdisciplinary team.

Ideal candidates will have RN licensure in good standing, Bachelor's in Nursing, CPR, and 1–2 years' hospice/public health experience. A valid driver's license and reliable vehicle are required.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing to practice.
• 1 year of professional nursing experience within the last 3 years OR a baccalaureate degree with 2 years' experience in public health, home care, or hospice nursing preferred.
• Must be licensed driver with insured vehicle.
• CPR certification (hands-on training required).

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments.
• Provide professional nursing care per the nursing process and physician's plan of care.
• Develop, implement and revise the care plan with patient/family involvement.
• Administer medications and treatments as prescribed.
• Coordinate patient care as Case Manager when assigned.
• Educate patients and families on self-care and disease management.
• Attend interdisciplinary group meetings and participate in quality improvement activities.
