Job summary

Your Health Organization is seeking a Registered Nurse to join our growing home-based care team serving Florence and Sumter, SC. The Continuous Care Nurse role is a critical part of the patient’s care team, delivering in-home, short-term bedside clinical care to stabilize symptoms and ensure comfort, dignity, and safety.

Responsibilities include coordinating with interdisciplinary teams, reviewing records, enrolling patients in Care Management, and documenting activities.

Qualifications

• Must be a licensed nurse with good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community/home settings is a plus.

Responsibilities

• Facilitate coordination of care with patient’s providers and team members.
• Review medical records and complete necessary consents.
• Enroll patients in Care Management and initiate a Plan of Care.
• Capture diagnoses with specificity and update care plans.
• Complete wellness visits and cognitive screenings as appropriate.
• Provide in-home bedside clinical care to manage symptoms and ensure comfort.
