Qualifications

• RN with Associates, Bachelors, or Masters in an accredited nursing program.
• 1+ years acute care experience; hospice/home health preferred.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Develop individualized care plans with patients and families.
• Perform 1:1 in-home assessments and updates as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties and coordinate seamless care transitions.
