Job summary

Responsibilities

• Collaborates with the IDG to assure palliative care evaluation and treatment.
• Educates patient and family about disease, self-care, and end-of-life care; trains staff as needed.
• Performs Face-to-Face visits at patients' residences.
• Interacts with attending and consulting physicians regarding palliative care.
• Provides clinical assessment results to hospice physician after encounters.
• Keeps up with pharmacology and palliative medicine knowledge.
• Applies hospice criteria knowledge.
