Job summary
Choice Health at Home is seeking a Hospice Nurse Practitioner to travel to patients' homes in the San Antonio, TX area. You will collaborate with the IDG to provide comprehensive palliative care and educate families on disease processes and end-of-life care.
The role requires active RN and APN licenses, with a focus on collaborative practice, pharmacology knowledge, and staying current with hospice criteria. This is a PRN position serving the San Antonio Hospice Branch.
Qualifications
- Current RN and Advanced Practice Nurse licenses.
- Hospice care or palliative medicine experience preferred.
- Ability to work in patients' homes and collaborate with a multidisciplinary team.
Responsibilities
- Collaborates with the IDG to assure evaluation and palliative treatment of terminal disease.
- Educates patient and family about disease process, self-care, and end-of-life care; trains staff as needed.
- Perform Face-to-Face visits at the patient’s residence.
- Interact with attending and consulting physicians regarding palliative care, in collaboration with hospice physicians.
- Provide clinical assessment results to hospice physician after encounters with patients.
- Maintain up-to-date knowledge in pharmacology and palliative medicine.
- Knowledge of hospice criteria.
