Choice Hospice, Inc. in Palestine, Texas is seeking a Nurse Practitioner to join our hospice team as a PRN provider. The NP will work within the IDG framework to evaluate and manage terminal illness with compassionate, patient-centered care.

The ideal candidate will have current RN and Advanced Practice Nurse licenses and thrive in an unstructured environment, providing education to families and collaborating with physicians to deliver high-quality end-of-life care.

Qualifications

• Current STATE RN and Advanced Practice Nurse License.

Responsibilities

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care.
• Perform Face-to-Face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Knowledgeable of hospice criteria.
