Sage Hospice, Primary and Palliative Care invites qualified registered nurses to join our home-based patient care team in the Phoenix, AZ area. You will provide skilled nursing services in patients' homes, coordinate with families and the care team, and help ensure comfort and quality of life for our clients.

We value compassionate caregivers, strong clinical skills, and the ability to support families facing serious illness. A state RN license and CPR certification are required.

Formación

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, cope with family emotional stress and diverse lifestyles.

Responsabilidades

• Administer skilled nursing care for clients of all ages in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Assume responsibility for coordination of care and patient/family support.
