Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current, unrestricted state RN license.
• CPR certification (hands-on).
• TB skin test within last 12 months.
• Reliable transportation.

Responsibilities

• Deliver and document skilled nursing care per care plan and physician orders.
• Observe and assess patient condition and report changes to supervisor/physician.
• Document activities, nursing actions and coordination of care promptly.
• Develop, implement and update the nursing care plan.
• Provide care using infection control measures per OSHA standards.
• Educate patient and family on disease, self-care and prevention.
