Job summary

Mental Health Association of Rochester seeks an experienced Registered Nurse to provide skilled nursing care in patients' homes. You will perform comprehensive assessments, administer treatments, manage wounds and IV therapy, and coordinate with an interdisciplinary team to support recovery and safety.

Responsibilities include updating care plans, documenting in OASIS and EHR systems, and educating patients and families to promote self-management and independence.

Qualifications

• Current RN licensure with strong clinical assessment and critical thinking skills.
• Proficiency in medication administration, wound care, and IV management.
• Strong documentation skills for OASIS and EHR systems.
• Excellent communication and patient/family education abilities.
• Commitment to patient-centered care, teamwork, and clinical excellence.

Responsibilities

• Perform comprehensive patient assessments and ongoing monitoring.
• Administer treatments and medications safely and accurately.
• Manage wounds, IV therapy, and other clinical interventions.
• Develop and update individualized care plans to support recovery, safety, independence, and comfort.
• Complete accurate OASIS and EHR documentation.
• Educate patients and families to promote understanding and self-management.
• Collaborate with physicians and an interdisciplinary team to coordinate care.
