Job summary

Centrix Health Resources is seeking a licensed Registered Nurse to provide skilled nursing services in patients' homes across Arizona. Responsibilities include executing physician orders, assessing patient needs, and documenting care plans with adherence to agency policies.

You will initiate Start of Care assessments, monitor patient progress, and collaborate with physicians and care teams to adjust plans as needed.

Qualifications

• Currently licensed as a Registered Nurse to practice in Arizona.
• Or holds a multi-state license with a valid compact state license for up to 30 days.

Responsibilities

• Provide skilled nursing services per physician orders and Plan of Care.
• Assess, document, and monitor patient health and plan of care.
• Perform start-of-care assessments, medication profiles, and care plans.
• Communicate with staff and physicians to update orders.
• Maintain accurate patient records and ensure timely documentation.

Education

Registered Nurse license (AZ) or multi-state compact license
