Job summary

21st Century Home Health Services in the Bay Area seeks a Home Health Field Nurse to provide skilled in-home nursing care across multiple counties, including Napa. You will follow a plan of care, document promptly, and support patients and families with education and self-care techniques.

The role requires an active CA RN license, BLS, and a valid CA driver’s license. You will work with an interdisciplinary team to coordinate services and ensure quality outcomes for diverse patients.

Qualifications

• One year of nursing experience; waiver may apply for internal candidates or new grads.
• One year of home health care experience preferred.
• Experience with high-acuity patients is a plus.
• Interdisciplinary team collaboration expected.

Responsibilities

• As Home Health Field Nurse you will provide hands-on patient care in the home within RN scope.
• Implement a plan of care and document within 24–48 hours.
• Educate patients and families about diagnoses and treatments.
• Coordinate with other disciplines to arrange services and reports.
• Maintain patient privacy and records; participate in weekly case conferences.
• Attend on-call duties as required by weekend rotation.
