Job summary

21st Century Home Health Services (21HHS) is seeking an entry-level Field Registered Nurse to develop core nursing skills in a home-health environment. You will work under the Case Management RN, visiting patients in their homes and contributing to coordinated care plans.

The role emphasizes hands-on nursing, documentation, and patient education, with a schedule Tue–Sat or Sun–Thu. California RN licensure and BLS are required, and a driver’s license is needed for travel to patient homes.

Qualifications

• Current CA Registered Nurse License is required.
• BLS Certification required.
• At least 1 year of recent nursing experience; home health experience preferred.
• Must be self-directed with strong communication and organizational skills.
• Proficiency with patient documentation and care plans.

Responsibilities

• Provide hands-on patient care within RN scope (home health).
• Implement and document plan of care directed by Case Management RN.
• Record timely documentation within 24–48 hours.
• Educate patients and families about diagnoses and care plans.
• Coordinate with HHA, OT, PT, MSW, ST for services.
• Participate in weekly Case Conferences and on-call duties.
• Maintain patient privacy and professional standards.
• Complete Start of Care assessments and care plans with Case Manager.
