Job summary

21st Century Home Health Services in California is seeking a dedicated Home Health Field Nurse to join our team. You will provide hands-on patient care in patients' homes, within the RN scope, and support the Case Management team in executing the plan of care.

The role requires a CA RN license, BLS, a valid driver's license, and strong communication skills. Previous home health experience is preferred, and you will participate in on-call duties and multidisciplinary collaboration.

Qualifications

• One year of nursing experience (may be waived for internal candidates/new grad posting).
• CA RN license current and active.
• BLS certification required.
• Driver's license with acceptable driving record.
• Strong interpersonal and communication skills.

Responsibilities

• Provides hands-on patient care in the home within RN scope.
• Implements and documents the plan of care directed by the Case Management RN.
• Participates in coordination of home health services with interdisciplinary team.
• Keeps accurate documentation within 24–48 hours of care.
• Participates in on-call duties and weekend rotations.
