Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to perform in-home health evaluations. The role involves engaging with patients in their homes, reviewing medical histories, performing physical evaluations, and coordinating with health plans for appropriate care.

You will use an iPad and follow a clinical workflow during visits. Qualified MDs/DOs must hold active licenses; multi-state licenses are preferred.

Qualifications

• Active, unrestricted license(s) in the coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Join a national network of clinicians delivering in-home health evaluations.
• Travel locally and within licensed states as needed.
• Conduct in-home visits and virtual visits as required.
• Provide Signify Health services including diagnostic and preventive assessments.
