Ascension at Home, Together with Compassus, is seeking a Home Health Occupational Therapist to empower patients to live safely at home. You will work one-on-one with patients in their homes, helping adaptations and activity participation to improve quality of life.

Minimum qualifications include a BS in OT, state license, driver’s license, CPR, and TB/Hepatitis clearance, plus at least two years of acute care or rehab experience. This role supports compassionate, patient-centered home care.

Qualificações

• BS in Occupational Therapy required.
• Current State License as an Occupational Therapist.
• Valid Driver’s License required.
• Current CPR and TB/Hepatitis consent or declination.
• Two years’ experience as an OT in acute care or rehabilitation setting.

Responsabilidades

• Work one-on-one with patients in their home to provide OT services.
• Help patients and families adapt environments, modify tasks, and use equipment to improve participation in daily activities.
• Collaborate with care teams to support patient safety, independence, and quality of life.
