Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations, engaging with people in the comfort of their own homes and bridging gaps in care.

The role involves building trusted relationships, traveling locally to licensed states, performing comprehensive clinical assessments, and delivering diagnostic and preventive services as needed, using an iPad and streamlined workflow.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Join a national network of purpose-driven clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with the right care for their needs.
• Collaborate with Signify Health as a clinician.
• Travel locally within licensed states as needed.
• Conduct virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated (e.g., spirometry, DEE, etc.).
