Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members across the United States. Each visit lasts up to one hour and focuses on health history, medications, adherence, and risk factors to close care gaps.

The role involves collaboration with physicians, case managers, and other clinicians, with services at patients’ homes or Skilled Nursing Facilities. Excellent communication and NP licensure are required.

Qualifications

• Master’s degree in nursing and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in relevant NP specialty.
• Strong communication skills and computer literacy.

Responsibilities

• Conduct adult/geriatric in-home health assessments (up to 1 hour) including medical history and medication review.
• Administer/order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and other care team members.
• Provide care at member's residence or SNF and document findings per Matrix protocols.
