Job summary

Anuco Home Health Care LLC is seeking compassionate Licensed Practical Nurses to provide intermittent home health services in Chicago and surrounding communities. You will operate under RN supervision, ensure accurate documentation, and support patients and caregivers in self-care techniques.

The role emphasizes home health experience, strong clinical skills, and timely charting to maintain quality patient care.

Qualifications

• Current, unrestricted Illinois LPN license.
• Graduate of an approved practical nursing program.
• Minimum of one year nursing experience preferred.
• Home health experience strongly preferred.
• Axxess Home Health EMR experience preferred.
• Current CPR or BLS certification.
• Valid driver’s license and reliable transportation.
• Professional liability insurance as required.
• Strong observation, communication, organization, and time-management skills.
• Ability to travel to patient homes and perform essential duties.

Responsibilities

• Provide nursing services, treatments, and preventive or rehabilitative procedures per physician orders and plan of care.
• Implement the nursing plan under RN supervision.
• Administer medications and treatments within LPN scope.
• Monitor vital signs and treatment response.
• Report significant changes to supervising RN and Clinical Manager.
• Prepare equipment and maintain infection-control techniques.
• Educate patients and caregivers on medications and care.
• Maintain accurate records of observations and care.
• Accept orders with RN review or cosignature.
• Notify office of scheduling changes.
• Coordinate with RN and team on patient needs.
