Job summary
Anuco Home Health Care LLC is seeking compassionate Licensed Practical Nurses to provide intermittent nursing services in patients' homes under the direction of a Registered Nurse, across Chicago and surrounding communities.
The ideal candidate has home health experience, strong clinical and communication skills, proficiency with Axxess Home Health EMR, and a commitment to timely documentation and patient-centered care.
Qualifications
- Current, unrestricted Illinois LPN license.
- Graduate of an approved practical nursing program.
- Minimum of one year of nursing experience preferred.
- Home health experience strongly preferred.
- Axxess Home Health EMR experience preferred.
- Current CPR or BLS certification.
- Valid driver’s license and reliable transportation.
Responsibilities
- Provide nursing services, treatments, and preventive procedures per physician orders.
- Administer medications and treatments within LPN scope.
- Monitor vital signs and patient condition; report changes to RN.
- Document observations and care provided accurately in time.
- Educate patients and caregivers on medications, safety, and plan of care.
- Coordinate with RN and interdisciplinary team on patient needs.
- Provide on-call, weekend, or holiday coverage as assigned.
