Job summary

Anuco Home Health Care LLC is seeking compassionate Licensed Practical Nurses to provide intermittent in-home nursing services in Chicago and surrounding communities. The role reports to the Clinical Manager and requires strong clinical and communication skills with a focus on timely documentation and coordination with the RN team.

The ideal candidate has home health experience, CPR/BLS, a current IL LPN license, and the ability to travel to patient homes.

Qualifications

• Current, unrestricted Illinois LPN license.
• Graduate of an approved practical nursing program.
• Minimum of one year nursing experience preferred.
• Home health experience strongly preferred.
• Axxess Home Health EMR experience preferred.
• Current CPR or BLS certification.
• Valid driver’s license, reliable transportation.
• Professional liability insurance as required.
• Strong observation, communication, organization, and time-management skills.
• Ability to travel to patient homes and perform essential duties with or without reasonable accommodation.

Responsibilities

• Provide nursing services, treatments, and preventive or rehabilitative procedures according to physician orders and the established plan of care.
• Implement the nursing plan of care under the direction and supervision of a Registered Nurse.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Monitor vital signs, symptoms, medication effectiveness, treatment response, and the patient’s physical or emotional condition.
• Prepare equipment and supplies and maintain appropriate infection‑control and aseptic techniques.
• Assist patients and caregivers in learning appropriate self‑care techniques.
• Educate patients and caregivers regarding medications, treatments, safety, and the plan of care.
• Maintain accurate and complete records of observations, treatments, education, and care provided.
• Accept and document orders as permitted, with required RN review or cosignature.
• Coordinate with the RN and interdisciplinary team regarding patient needs and referrals.
• Provide on‑call, weekend, or holiday coverage when assigned.
• Protect patient confidentiality and comply with HIPAA and agency policies.
