Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP/PA) to provide in-home health evaluations, delivering care directly at members’ homes. The role supports bridging gaps in care and includes a flexible schedule with travel within licensed states.

The position emphasizes thorough health assessments, patient education, and a patient-centered approach while using an iPad and streamlined workflows. Week-to-week hours average 29.5.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
