Job summary

Signify Health is seeking a Part Time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting members in their homes and bridging gaps in care. This role emphasizes compassionate, personalized health assessments delivered at the person’s doorstep.

The position involves 29.5 hours per week, travel within licensed states, and virtual visits as needed, with a comprehensive benefits package for part-time employees.

• Active, unrestricted license(s) in coverage area SEATTLE SNOHOMISH
• Board certification (required)

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.

• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).
