Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive health assessments in members’ homes across the United States. Each visit lasts up to one hour and focuses on medical history, current health, medication adherence, and social determinants of health.

You will review care plans, order testing, and work closely with PCPs to close care gaps. Positions offer flexible scheduling, competitive pay based on assessments completed, and opportunities for full-time roles with

Competenze

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in relevant NP specialty (Adult, Gerontology, Family, Emergency, Acute Care).
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver’s license for state of residence.
• Auto insurance coverage as required by state.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, and documentation per Matrix guidelines.
• Order and administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Educate patients and coordinate follow-up with PCP.
• Provide services at members’ residences, including homes or SNFs.
