Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes across our national network. Visits last up to one hour and focus on medical history, adherence, and risk factors to close care gaps.

Providers administer required tests, collaborate with physicians and care teams, and deliver education and follow-up in a home setting, including potential SNF visits. Competitive pay and flexible scheduling are offered.

Competenze

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred, new grads encouraged to apply!
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.

Mansioni

• Conduct adult/geriatric health assessments including medical history, diagnoses, and treatment plans.
• Administer and order point-of-care and lab-based testing as needed.
• Coordinate with physicians, case managers, social workers, family members, and care teams.
• Collaborate with PCP on patient education and follow-up.
• Provide services at member residences, including homes and Skilled Nursing Facilities.
