Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments across assigned regions. You will evaluate medical history, medications, and social factors, while coordinating with physicians and case managers to close care gaps.

The role focuses on high-quality patient education, preventive care, and timely documentation, with flexible scheduling and opportunities for full-time advancement.

Qualifications

• Master's Degree or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in relevant Adult/Family gerontology specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license.
• Automobile insurance coverage or ability to obtain.

Responsibilities

• Conduct adult/geriatric health assessments including med history, diagnoses, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP on patient education and follow-up.
