Job summary

FOX Rehabilitation in Charlottesville, VA is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. You’ll evaluate, plan, and treat to improve mobility and independence while coordinating with an interdisciplinary team.

Clinicians enjoy autonomy to set schedules, potential telehealth components, and a commitment to meaningful patient outcomes in a clinician-driven practice.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and longevity.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
