Job summary
FOX Rehabilitation is expanding in St. Matthews, KY, seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes or senior living communities.
You will conduct evaluations, develop plans, and provide evidence-based therapy, promoting mobility and independence through our Geriatric House Calls model. Clinicians enjoy autonomy, flexible scheduling, and opportunities to use telehealth to reach more patients while collaborating with PTs, OTs, SLPs, and Fitness
Qualifications
- Valid Physical Therapy license in the state(s) of practice.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Responsibilities
- Provide one-on-one physical therapy to older adults in homes or senior living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support wellness, mobility, and independence.
- Document effectively within FOX’s EMR system in compliance with regulations.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth / virtual care when appropriate to extend access.
