Job summary

FOX Rehabilitation seeks licensed Physical Therapists to provide one-on-one care to older adults in Greater Boca Raton, FL, including private homes and senior living communities. You will evaluate, plan, and deliver personalized therapy while documenting in FOX's EMR and collaborating with a diverse care team.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
