Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in Boca Raton, FL, in private homes or senior living communities. You will assess, treat, and document patient progress while supporting independence.

Role offers autonomy to manage your own schedule, opportunities for telehealth, and collaboration with an interdisciplinary team to promote mobility and aging in place for older adults.

Qualifications

• Must hold a valid Physical Therapy license in the state or eligibility to obtain one
• Basic computer and EMR proficiency required
• Current CPR certification is required

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility, and independence
• Document in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
