Job summary

FOX Rehabilitation is growing in Washington, D.C. and seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and senior living communities.

Work autonomously with a flexible schedule, evaluate patients, develop plans of care, and provide evidence-based therapy. Telehealth may be incorporated to extend access and continuity of care, while collaborating with an interdisciplinary team.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency. CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
