Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one in-home and senior living care in New Rochelle, NY. You will evaluate patients, develop plans of care, and deliver evidence-based therapy to improve mobility and independence.

Flexibility to create your schedule, potential telehealth options, and collaboration with PT/OT/SLP teams help deliver high-quality care to older adults in their homes and communities.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
