FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one PT services in older adults’ homes and in senior living communities around Port St. Lucie, Florida. You’ll evaluate, plan, and deliver evidence-based treatments with autonomy and flexible scheduling.

The role emphasizes aging-in-place goals, collaboration with an interdisciplinary team, and the option to incorporate telehealth to extend access and continuity of care for patients.

Competenze

• Requires PT license in state(s) of practice or eligibility to obtain
• Degree from an accredited PT program
• Basic computer and EMR proficiency

Mansioni

• Provide one-on-one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver sessions focusing on wellness, mobility, independence and function
• Document in FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers and staff to promote aging in place
• Provide telehealth/virtual care when appropriate
