FOX Rehabilitation is expanding in Jefferson, IA, seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. You’ll develop personalized plans, provide evidence-based therapy, and document in FOX’s EMR while collaborating with an interdisciplinary team.

You’ll enjoy autonomy, flexibility, and meaningful patient outcomes, with options for telehealth and a schedule you build.

Qualificações

• Must hold a valid Physical Therapy license in the state of practice or be eligible to obtain one
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that promote wellness, mobility and independence
• Document effectively within FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
