Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in Walcott, IA, including private residences and senior living communities. Clinicians enjoy autonomy, flexible schedules, and the option to incorporate telehealth as part of the Geriatric House Calls™ model.

Responsibilities include patient evaluations, care planning, and delivering evidence-based therapy while documenting in the EMR system and collaborating with an interdisciplinary team to promote

Qualifications

Must hold valid Physical Therapy license in the practicing state.
Degree from an accredited Physical Therapy program.
Current CPR certification.

Responsibilities

Provide one-on-one physical therapy to older adults in home or community settings.
Perform evaluations, develop plans of care, and deliver evidence-based treatment.
Document in EMR per regulatory standards and maintain accurate records.
Collaborate with an interdisciplinary team to coordinate care.
Educate patients and caregivers to promote safety and aging in place.
