Qualifications

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document in EMR per standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
