Job summary

Fox Rehabilitation in Jackson Heights, NY is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients’ homes or senior living communities. The role emphasizes autonomy, schedule control, and meaningful patient outcomes through our Geriatric House Calls model.

Clinicians may incorporate telehealth to extend access and continuity of care, with opportunities for performance-based growth and in-home as well as community-based practice.

Qualifications

Licensed Physical Therapist in NY or eligibility to obtain licensure.
Degree from an accredited Physical Therapy program.
Basic computer skills and EMR proficiency.
Current CPR certification.

Responsibilities

Provide one-on-one physical therapy to older adults in homes or community settings.
Perform evaluations and develop individualized care plans.
Deliver sessions to improve mobility, independence and function.
Document in the EMR per regulatory standards.
Collaborate with an interdisciplinary team (PT, OT, SLP, fitness).
Educate patients and caregivers on safety and aging in place.
Offer telehealth/virtual care when appropriate.
