Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care in patients' homes and senior living communities across Scottsdale, AZ. You will assess, plan, and provide evidence-based therapy with options for telehealth to extend access and continuity of care.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions that support wellness, mobility, independence.
• Document effectively in FOX's EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
