Pathway Geriatrics PLLC, a nurse practitioner‑owned medical practice headquartered in Redmond, WA, provides in-home palliative primary care to homebound older adults in the Greater Seattle area. You will serve as the primary care provider for a large panel, delivering care in AFHs, assisted living, and private homes.

The position is 100% field-based with weekly team meetings at the Redmond office. Typical hours are 9:00 AM to 5:00 PM, Monday through Friday.

Responsibilities

• Serve as the PCP for a panel of about 200 homebound older adults in the Seattle Eastside region, with most clients in Adult Family Homes (AFHs) or private residences.
• Facilitate each client's medication and chronic care management between in-person appointments.
• Manage urgent situations remotely with support from a local network of in-home diagnostic services (mobile imaging, ultrasound, phlebotomy, urinalysis).
• Visit between one and five homes per day, typically every 5–6 weeks depending on medical necessity and provider judgment.
