Job summary

Springwell is seeking a Case Management professional to help older adults age-in-place and assist individuals with disabilities in the community. Compensation is $28.21/hr for a 37.5 hour work week, with benefits and growth opportunities.

The role involves collaborating with an interdisciplinary team, conducting home visits, performing comprehensive assessments, and contributing to person-centered care plans to support independence and dignity.

Qualifications

• Bachelor's Degree or higher.
• Associate's Degree with 3+ years' elder/case management experience or 50% Bachelor's coursework with 3-year completion plan.
• Strong desire to work with seniors and individuals with disabilities.
• Excellent organizational skills and ability to manage change with a positive attitude.
• Experience using computers, data entry, and running reports.
• Reliable car, valid driver's license, and safe driving record.

Responsibilities

• Work collaboratively as part of an interdisciplinary team providing in-home services to older adults and individuals with disabilities.
• Make home visits, conduct comprehensive assessments, and participate in development of person-centered care plans.
