Job summary

Habitat Health is seeking a Home Care Coordinator to join our interdisciplinary team in delivering in-home and clinical care to participants in Sacramento, CA. You will assess needs, develop care plans, and coordinate home care services to support safe, effective care in the home setting.

The role requires at least 2 years of clinical home care experience, strong communication, and proficiency with electronic health records. A valid driver’s license and reliable transportation are required.

Qualifications

• Minimum of two (2) years of clinical experience in home care or related healthcare setting.
• Strong clinical judgment and critical thinking.
• Excellent interpersonal and collaboration skills.
• Proficiency with electronic health records and healthcare documentation systems.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate care needs and safety.
• Develop, implement, and update participant care plans with IDT.
• Coordinate home care services to align with individual care plans and goals.
• Maintain accurate authorizations and documentation in EHRs.
