Job summary

Advantmed is seeking a licensed Nurse Practitioner in Corunna, Michigan, to conduct in-home wellness assessments for Medicare members. This flexible PRN role allows you to choose your own weekday availability. Responsibilities include conducting wellness visits, educating members, and documenting in the EMR. An active NP license and 3+ years of patient care experience are preferred. Competitive pay includes $100 per completed assessment plus mileage. Strong operational support will be provided for your success.

Qualifications

• Active, unencumbered NP license in the applicable state.
• 3+ years of patient care experience preferred.
• Comfortable with EMR systems and independent work.

Responsibilities

• Conduct in home Annual Wellness Visits and chronic condition assessments.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care.
