Job summary

The Connection seeks a Family Support Specialist 1 to provide intensive in-home case management for families facing homelessness or risk. You will help clients achieve housing, employment, and service goals by coordinating resources and collaborating with treatment providers and community partners.

Qualifications include a Bachelor's Degree with at least 2 years of relevant experience, strong interpersonal and communication skills, and a valid driver's license.

Qualifications

• Strong interpersonal and communication skills.
• Experience with case management.
• Familiarity with Motivational Interviewing (preferred).
• Valid Driver’s License.

Responsibilities

• Weekly in-home visits to support clients in developing and achieving housing, employment, and treatment goals.
• Identify resources and referrals for housing, eligible benefits, substance use, and mental health services.
• Regular collaboration with stakeholders such as DCF, treatment providers, landlords, and internal staff.
