Qualifications

Responsibilities

• Conduct multi-level family assessments and develop service/treatment plans.
• Ensure documentation meets quality standards and is timely.
• Provide home-based services and family-centered supports.
• Train and supervise staff; perform performance reviews.
• Coordinate with Indiana DCS and community resources.
