Qualifications

Responsibilities

• Assess needs to establish short- and long-term goals.
• Connect families to housing, employment, and social services.
• Provide mentorship, advocacy, and support to ensure client success.
• Assist families in developing life skills, coping skills, and healthy relationships.
• Document sessions, case notes, and reports within required timelines.
• Maintain safety and treatment plans in Case Management Pro.
• Coordinate with team members and participate in supervision.
